One provincial organization that arranges home and community care, long-term care home placement and help finding services in the community. Based on your assessment, home care can include nursing, personal support (such as bathing and dressing), physiotherapy, occupational therapy, speech-language therapy, social work, nutrition counselling, medical supplies and equipment, and palliative care. It was formerly called Home and Community Care Support Services.
Who qualifies
Ontario residents of all ages with a valid OHIP card can get an assessment by a care coordinator, who decides which services you qualify for; services are for people who need care at home, at school or in the community. Anyone can make a referral with your consent: you, a family member, a friend, a caregiver, a neighbour or your doctor.

You can call for yourself, or a family member, friend, caregiver or doctor can refer you with your consent. Answer a few questions about your needs. You will be connected with a care coordinator, or referred to a community program that fits your needs. The care coordinator assesses your health (sometimes in your home) and makes a care plan with you. Then a care provider comes for the first visit. For long-term care, all applications and admissions are arranged by Ontario Health atHome: your care coordinator helps you apply, and you can choose up to five homes.
